The work helped participants connect data with the real conditions they see at the workplace. Rather than treating RCA as a form-filling exercise, the session pushed the team to observe current conditions, clarify facts, and use structured analysis before jumping to conclusions or countermeasures.
Practical RCA approach
The day combined on-site observation with facilitated discussion. Participants reviewed workplace conditions, discussed possible causes, and linked findings back to improvement actions. This format helped executives and officers practise fact-based thinking using actual operational context, not generic classroom examples.
